DO NOT keep medication in your
locker or backpack. If it is necessary for
you to take medication during school hours, take it to the office. We
must have a signed Medication Permission form for any type of
medication including over-the-counter.
All medication, prescription
and over-the-counter, must follow California State Health and Safety Codes and
district policy. In most instances, students with inhalers and
epi-pens are allowed to carry them with the proper paperwork. All other medications must be kept in a
locker in the school office. School
personnel will give your child medication at school if the following
guidelines are met:

Permission Form for Administering Medication in School. You may
obtain a form at the school office or click below to print a copy.
2. Bring medication to the school office
in a pharmacy-labeled bottle that includes student name, medication name,
