 MEDICATIONS AT SCHOOL

 
 

DO NOT keep medication in your locker or backpack.  If it is necessary for you to take medication during school hours, take it to the office.   We must have a signed Medication Permission form for any type of medication including over-the-counter. 

All medication, prescription and over-the-counter, must follow California State Health and Safety Codes and district policy.  In most instances, students with inhalers and epi-pens are allowed to carry them with the proper paperwork.  All other medications must be kept in a locker in the school office.  School personnel will give your child medication at school if the following guidelines are met:

1.    Parent and physician complete the Permission Form for Administering Medication in School.  You may obtain a form at the school office or click below to print a copy.   

2.    Bring medication to the school office in a pharmacy-labeled bottle that includes student name, medication name, dosage and time to be given.  Over-the-counter medications must be in the original bottle. 

3.    The Permission Form for Administering Medication in School must be updated yearly.​
